Form 77 Achievement Week Award packages must be emailed to the Second District Achievement Week Committee at achievementweek@opp2d.org by the due dates announced.  The 2021-2022 Achievement Week application period and closing dates for the Second District awards have not yet been announced.  The 2021-2022 awardees will be announced at at the 75th Second District Conference in 2023.

  • Individual Awards are due by February 10, 2023; and, 

  • Chapter Awards are due by March 10, 2022.

Links to access the Form 77, Achievement Week Award Applications, last revised November 1, 2019, are provided herein below.  Note that the Forms 77 are subject to change for the 2021-2022 evaluation period, as determined by the International Achievement Week Committee.  Any new editions of the Forms 77 will be published and communicated as soon as available.

TRAINING

Form 77 training sessions are provided with notice given and issued through the DKRS to all and/or directly to the Chapter Achievement Week Chairman where appropriate.  Requests for training and additional information should be directed to the Second District Achievement Week Committee Chairman via achievementweek@opp2d.org.

Additional training may be made available via Second District workshops announced via the DKRS and/or the Second District Achievement Week Chairman, and/or may be supplemented via training videos published to this page.
